ANO ANG COMA?

‘Eto usapang doktor na. Ang coma ay isang deep state of unconsciousness.
Coma is different from sleep because the person is unable to wake up.
It is not the same as brain death. The person is alive, but they cannot respond in the normal way to their environment.
They do not respond to pain, light, or sound in the usual way, and they do not make voluntary actions. Although they do not wake up, their body follows normal sleep patterns. Automatic functions, such as breathing and circulation, normally continue to function, but the person’s thinking ability is sup-pressed. In short, hindi rin kayang magmura.

MGA DAHILAN NG COMA

COMATOSE-2Coma may occur for various reasons, such as intoxication, a disease or infection that affects the central nervous system (CNS):
• Traumatic injuries to the head.
• Some types of infection.
• Diabetes whether hyperglycemia or hypoglycemia.
• Hypoxia or lack of oxygen.
• Tumors in the brain.
• After a heart attack, stroke, or near drowning.
• Infections like encephalitis or meningitis
• Severe inflammation of the brain or spinal cord.
• Toxins like carbon monoxide or lead.
• Drugs and alcohol overdose.

SINTOMAS NG COMA

If a person enters a coma, this is a medical emergency. Rapid action may be needed to preserve life and brain function.
A coma often lasts for a few days or weeks, but rarely, it can last for several years.
• Closed eyes
• Depressed brainstem reflexes
• Pupils not responding to light
• No responses of limbs
• No response to painful stimuli
• Irregular breathing

SA EMERGENCY ROOM

COMATOSE TESTHindi ho kinukuhanan ng video at minumura ang mga doktor, gaya ng isa ring trending na balita sa fb.
May tinatawag ho kasi ang karamihan sa mga ospital na ‘triage’. Dito ho inaalam ng mabilisan kung gaano ka-emergency ang sitwasyon ng pasyente at kung saang espesyalista ipadadala. Makabubuti ho na huwag na aabalahin ang mga staff dito para makapagtrabaho ng wasto. Doctors will:
• Check the patient’s reflexes.
• Check response to painful stimuli.
• Shine a light in the eyes for pupil size.
• Observe breathing patterns.
• Check the skin for signs of any bruises due to trauma.
• Doctors may speak loudly to check hearing.
• Press on the angle of the jaw or nail bed for response.
• May squirt ice-cold into ear canals to observe eye reactions.

LABORATORY TESTS

• Complete blood count
• Serum Electrolytes
• Blood sugar test
• Thyroid, kidney and liver function tests
• Lumbar puncture to check for brain infection
• Computerized tomography (CT) scan to show hemorrhage, tumors or stroke
• Magnetic resonance imaging (MRI) to detect brain tissue damage
• Electroencephalography (EEG to measure electrical activity of the brain

ANO ANG GLASGOW SCALE?

Ang Glasgow Coma Scale (GCS) ay isang pamamaraan para masuri at mabigyan ng accurate na evalua-tion ang isang tao tungkol sa kaniyang brain damage.
Tama ho kayo, dapat pati sa mga politiko at reporters na may ‘brenda’ (brain damage) dapat ginaga­mit din ito. The scale was published in 1974 by professors Graham Teasdale and Bryan J. Jennett, at the University of Glasgow’s Institute of Neurological Sciences.
• Eyes: Scores range from 1 to 4, where 1 is when a person does not open their eyes, 2 is when they open their eyes in response to pain, 3 is when they open them in response to voice, and 4 is when they open them spontaneously.
• Verbal: Scores range from 1 to 5, where 1 means the person makes no sound, 2 means they mumble but cannot be understood, 3 is when they utter inappropriate words, 4 is when they speak but are confused, and 5 is normal communication.
• Motor Reflexes: Scores range from 1 to 6, and 1 to 5 describe a person’s response to pain. A person who scores 1 makes no movement, 2 is when they straighten a limb in response to pain, 3 is when they react in an unusual way to pain, 4 is when they move away from pain, and 5 is when they can pin-point where the pain is. A score of 6 means the person can obey commands.
A score of 8 or less overall indicates a coma. If the score is from 9 to 12, the condition is moderate. If the score is 13 or more, the impairment to consciousness is minor.
